This single guided application replaces the 7-form paper packet, checks your household's eligibility against the Board's published financial hardship thresholds, and lets you securely upload bills, cost estimates, and income verification in one place.

How to apply

  1. Check Eligibility & Select Your Need

    Answer three quick questions to confirm your household meets the Board's financial hardship thresholds and select whether you need help with unpaid medical bills (Form A-2), upcoming care (Form A-2C), or both.

  2. Complete Your Guided Application & Upload Documents

    Enter your patient and household income details (Forms A-1 and A-3) and upload photos or PDFs of your medical bills, treatment cost estimates, and income proof.

  3. Sign Authorizations & Track Your Case Status

    Complete the provider billing release and communication authorizations (Forms A-4, A-5, and A-6) online. Once submitted, NewPath Medical will review your case and notify you of the Board's decision.
